Development of over 9500 Lockout-Tagout (LOTO) procedures for all
industrial equipment. |

APS' Mission: |
To
create a procedure set that raises the bar on LOTO usability, accuracy,
and reliability, incorporating human factors in a graphical format.

The Challenges: |
Sources of technical data such as P&ID drawings, floor plans, equipment
technical drawings and labeling of equipment and controls was
inconsistent, outdated or missing. Existing LOTO documents were not
maintained and included a very high percentage of incorrect data.
Subject matter experts were often unavailable due to operational
requirements. |

APS' Approach: |
APS
formed a project team of experienced industrial operations and
maintenance training specialists. The team worked with the client
personnel including management, engineering, safety, maintenance
technicians and plant operators to obtain technical information. APS'
training specialists located and analyzed designated equipment. LOTO
documents were developed and coordinated with plant personnel. Each
completed LOTO document includes diagrams of equipment locations and
lock points, detailed description of procedure steps, and photographs to
illustrate the procedure. |

The Results: |
Project was completed
on time and under budget. Over 9,500 procedures were developed,
validated, and approved.
